Spouse had something like this happen to him yesterday. Then a co-worker ran into the problem today. This is on two different TWiki installations.

In both cases, when the page was saved and, in the saving, a partial duplicate of the rendered HTML source of the page was added at the bottom of the TWiki .txt file

Here's some of what I see at the end of my file. My co-worker's TWiki name has been obuscated as CoWorkerName

</textarea><script type="text/javascript">initTextAreaHeight();</script>
<div class="patternSigLine">
<span class="patternSig twikiLeft"><label for="sig" class="twikiGrayText">Your signature to copy/paste:&nbsp;&nbsp;</label><input class="twikiInputField" size="41" name="sig" type="text" readonly="readonly" title='Select, copy and paste your signature text to the bottom of your entry' id="sig" value="-- Main.CoWorkerName - 08 Apr 2008" /></span><!--/patternSig-->
<div class="twistyPlugin twikiMakeVisibleInline"> <span id="textareabuttons1show" class="twistyStartShow twistyTrigger twikiUnvisited twistyHidden twistyInited1"><a href="#"><span class="twikiLinkLabel twikiUnvisited"></span></a> </span> <span id="textareabuttons1hide" class="twistyStartShow twistyTrigger twikiUnvisited twistyInited1"><a href="#"><span class="twikiLinkLabel twikiUnvisited"></span></a> </span>  </div><!--/twistyPlugin twikiMakeVisibleInline--> <div style="display:inline;" class="twistyPlugin"><div id="textareabuttons1toggle" class="twikiRight twistyStartShow twistyContent twikiMakeVisibleBlock twistyInited1">
<div style="text-align:left;">
<a href="#"><img width="16" alt="Use monospace font" title="Use monospace font" align="top" src="/pub/TWiki/TWikiDocGraphics/monospace.gif" height="16" border="0" onclick="setEditBoxFontStyle('mono'); return false;" /></a> <a href="#"><img width="16" alt="Use proportional spaced font" title="Use proportional spaced font" align="top" src="/pub/TWiki/TWikiDocGraphics/proportional.gif" height="16" border="0" onclick="setEditBoxFontStyle('proportional'); return false;" /></a>

The original page contains a form and in the resulting weirdness, that form is duplicated as part of itself. I'll attach a screenshot.

WYSIWYG or raw? Any unusual plugins? What codebase (you list 4.0.5 and 4.1.2 below)? Can you reproduce the problem when you disable the "extra" plugins?

No feedback from reporter for 3 months and noone can reproduce it

So No Action
