Does this site look broken?. Use the LitterTray web for test cases.

If the closed attribute does not exist, then action edit will create the attribute with a "BAD DATE FORMAT" value. For example, here's the original action:

%ACTION{ who="DiabJerius" due="19-May-2006" }% FOO

Here's how it looks after the topic is saved:

%ACTION{ due="19-May-2006" creator="Main.DiabJerius" uid="000003" state="open" created="17-May-2006" who="Main.DiabJerius" }% FOO

And here's how it looks after clicking on "edit" in the rendered action table and saving with no changes:

%ACTION{ closed="BAD DATE FORMAT see %TWIKIWEB%.ActionTrackerPlugin#DateFormats" due="19-May-2006" creator="Main.DiabJerius" uid="000003" state="open" created="17-May-2006" who="Main.DiabJerius" closer="" notify="" }% FOO

This may be harmless, but it's freakin' out the users.

Ok, this is also in TWiki:Support.ActionTrackerCloserEmpty, so this is a dupe. Searching for bugs/issues is a PITA with so many places to look.
