If the
closed
attribute does not exist, then action edit will create the attribute with a "BAD DATE FORMAT" value. For example, here's the original action:

Here's how it looks after the topic is saved:

And here's how it looks after clicking on "edit" in the rendered action table and saving with no changes:
%ACTION{ closed="BAD DATE FORMAT see %TWIKIWEB%.ActionTrackerPlugin#DateFormats" due="19-May-2006" creator="Main.DiabJerius" uid="000003" state="open" created="17-May-2006" who="Main.DiabJerius" closer="" notify="" }% FOO
This may be harmless, but it's freakin' out the users.
